Healthcare in Vallarta: How Public IMSS and Private Hospitals Work for Foreigners

One of the most consequential decisions a new resident makes is how to handle healthcare — and Mexico runs on essentially two tracks. The public system, IMSS (Instituto Mexicano del Seguro Social), is free at the point of care for those who qualify. Foreigners need legal residency to enroll. If you work for a Mexican employer, enrollment is automatic; if you do not, you can buy voluntary coverage for a monthly fee that depends on your age. There are important limits. IMSS does not cover a range of pre-existing conditions — including congenital and chronic degenerative diseases, malignant tumors, addictions, mental illness, and HIV. And its services are provided in Spanish only, so you will want a plan for the language gap. That is where the private side comes in. Vallarta has well-regarded private hospitals — names like CMQ, San Javier, and Joya come up often — and many local doctors trained abroad and speak good English. Plenty of residents carry private insurance or simply pay out of pocket for private care, which is generally far cheaper than in the US.
